Regular monitoring of blood glucose level is essential to treat type 1 Diabetes Mellitus. As different types of diabetes have different diagnosis methods depending on the population, The well known A1C test which is used to measure the plasma glucose concentration is also useful to monitor blood sugar level apart from it the well known proposed screening technique for diabetes patients is also useful as different population has different health issues regarding to diabetes.

Session on: Pathophysiology of Diabetes

Type2 diabetes mainly characterized by either peripheral insulin resistance or inadequate insulin secretion from the pancreatic beta cell. Insulin resistance decreases glucose transport into the muscle cells and increases the breakdown of fats. In type2 diabetes the reciprocal between a glucagon-secreting alpha cell and insulin-secreting beta cell is lost, as a result of it, hyperglycemia can occur. GWAS(Genome-wide association studies) have identified SNPs which are responsible for the dysfunction of the beta cells which leads to type 2 diabetes mellitus.

Endocrinology deals with the glands and the hormone which bring change in the body. Different glands like pituitary, thyroid, adrenals, ovaries, testicles, and pancreas come under it. Excess or less production of the endocrine hormones may lead to change different physiological and behavioral activities in the body like proliferation, growth, and development of tissues and its function, sleep, digestion, respiration, excretion, mood, stress, lactation, movement, reproduction, and sensory perception. The endocrine hormones also act as a signaling molecule in different pathways. The improper production or malfunction of those hormones cause a set of different metabolic diseases. Diabetes is a disease where the produced insulin is not working properly or the production of insulin is less. It also causes a lot of diseases associated with diabetes.

Session on: Pediatric Diabetes

The endocrine glands disorder in the early childhood which effects on physical growth and sexual development is the main cause of pediatric diabetes. The autoimmune destruction of beta cells in Islet of Langerhans causes the pediatric type 1 diabetes as the productions of insulin is very less. The 20-25% of pediatric diabetes children are obese and have a lifetime dependency on exogenous insulin sources. Cardiovascular and microvascular complications are very common to the children who are affected by type 2 diabetes. There are approximately 13000 children are diagnosed with diabetes each year in the USA. There is a rapid increase in prediabetes and type2 diabetes among children over 30 years.

The diabetes is associated with the nerve-damaging disorder causes micro-vascular injury which includes disruption of blood supply in vasa nervorum. The Diabetes Neuropathy affects peripheral nerves like sensory neurons, motor neurons. About 60-70% diabetic patients have neurological problems and it may cause autonomic neuropathy, focal neuropathy, proximal neuropathy etc.

Session on: Diabetic Retinopathy

The blood vessels of the retina which is thin light-sensitive inner lining in the back of the eyes are damaged in the diabetic retinopathy. There are two stages of diabetic retinopathy, non-proliferative retinopathy, and proliferative retinopathy. In proliferative retinopathy, the risk of hemorrhage into the vitreous or detachment of the retina is greater, and loss of vision can happen. Diabetic macular edema occurs at this stage. The high levels of blood glucose can damage the blood vessels in the retina in the early stage of non-proliferative retinopathy. As a result of it, the leakage of fluid can swell the retina and the vision becomes blurry. It can be treated with laser surgery when the central vision is threatened.

Session on: Diabetic Nephropathy

Due to diabetes, small blood vessels in the kidneys can be damaged and the blood cannot be cleaned properly. As a result of it, the water and salt concentration will be more in the body and causes weight gain and ankle swelling. The waste materials concentration can be high in the blood. The nerve damage due to diabetes can also make difficulty in emptying the bladder. An about 30 percent of type1 diabetic patient and 10-40 percent type2 diabetic patient suffers from kidney damage. The increased excretion of albumin can be a sign of diabetic kidney failure.

Obesity which is a global health issue has been characterized as a significant and growing health problem at present time. Physical activity, Healthy eating, and exercise are key lifestyle factors in managing weight. Stress, low-income levels, some medical conditions and certain medications can make a challenge in maintaining weight. About 1.4 billion people have overweighted of which a number of 500 million people are obese. The consumption of high-calorie food which has more sugar and fats can be a cause of overweight. The modern lifestyle has also given us a number of advances in technology which is also a cause of obesity. The lower income countries are also suffering from obesity as they undergo nutrition transition in which the consumption of animal source food, processed food, edible oil and sugar-sweetened drinks are gradually increasing. Diabetes especially type2 has a close relationship with the weight and over 90% of newly diagnosed type 2 diabetes is above their ideal weight. Having a proper diet and exercise will help in losing some weight which will prevent in developing diabetes.

The population-based studies have confirmed that there is an association between cancer and diabetes in the late 1960s. Recent studies have indicated that some cancer is very common in the type 2 diabetes patients. The risk of cancer in liver and pancreas are higher in diabetes affected patient because both the liver and pancreas are exposed to higher concentration of endogenously produced insulin. Factors which are related to diabetes may enhance the risk of liver cancer. The abnormal glucose metabolism also may cause pancreatic cancers.

Session on: Management of Diabetes

The main goal of diabetes management is to maintain the blood glucose level either by maintaining lifestyle management or by up taking insulin from outside. The target of the glucose management is to achieve the level of HbA1C in between 6.0 to 7.0, preprandial blood glucose 70-130 mg/dl and postprandial blood glucose <180mg/dl. These targets are individually based on the duration of diabetes, age/life expectancy, comorbidity, hypoglycemia awareness etc. Different risk factors such as cardiovascular diseases, coronary heart diseases are associated with it and controlling such risk factors may rise to secondary conditions of diabetes. Monitoring of cholesterol, LDL, HDL, and triglyceride may indicate hyperlipoproteinemia, which arises because of the treatment with hypolipidemic drugs. Maintaining the absolute proper limits of blood pressure helps to protect against different diseases like renal failure, cardiovascular complications. Diet and exercise play an important role in maintaining the blood sugar level.

Continuous glucose monitoring device is a tracking device of the blood sugar level on a daily basis. It works based on the sensor under the skin and gives lots of reading throughout the day. Depending on the blood sugar level it sends alert and helps the patients to take necessary steps. The obtained data will be useful for further medication. It leads to a tremendous improvement in the significant lowering of glycated hemoglobin (HbA1c) in adults with type I diabetes. Continuous subcutaneous insulin infusion is a technique to achieve intensive diabetic management in children, adolescent, and adult. The absorption of the modified insulin vary 19% to 55% in an individual and causes the variability of glucose in the blood where the absorption of the soluble, short-acting insulin use in CSII vary less than 3%.

The pharmacology of diabetes involves several drugs which have the common goal is to reduce the body glucose. Drug-like DPP-4 inhibitors, alpha-glucosidase inhibitors are common drugs in the diabetes treatment. The working of DPP-4 inhibitors is to block the action of the DPP-4 enzyme. This enzyme destroys the hormone called incretin. The incretin helps the body to produce insulin as per the body glucose. It also regulates the production of the glucose. The alpha-glucosidase inhibitors are used for the treatment of type2 diabetes mellitus. This drug prevents the digestion of the carbohydrate and thus it prevents the formation of the sugar molecules.

Beta cells release insulin hormone when the blood glucose rise up. The Artificial Beta Cells is subcutaneously inserted into the patient’s body and it should be replaced every few weeks. The Cells minimize the glucose controller of the body by secreting more insulin from the pancreatic cells. It works on the concept of sensor responsive insulin delivery system and it is also known as closed-loop insulin delivery system. Industries have turned towards the gut hormones as the new therapeutic drug for the diabetic patients. A group of an intestinal hormone known as incretins involved in the controlling of the blood sugar level. Researchers on the human skin cell give us a new therapy for treatment to tackle type1 Diabetes Mellitus. The human skin cell which is abundant and easily available can be reprogrammed to prevent diabetes.

Market Analysis

There are 425 million adults across the globe suffering from Diabetes Mellitus and 1 million children were affected with type 1 Diabetes. It is a disease which occurs when the pancreas produces little insulin or no insulin at all, or the produced insulin is not working properly and causes 4 million deaths. 90-95% diabetes is type 2 and it is the 7th leading causes of death. Nearly 21 million births were affected by diabetes during pregnancy. About 60-70% diabetic patients have a nervous system disorder. Based On a report from International Diabetes Federation in 2011 an estimated 366 million people had Diabetes Mellitus, by 2045 this number will rise to almost 629 million. In the United States 30.3 million people, or 9.4 percent of the population, have diabetes. 23.1 million people have been diagnosed with diabetes which is equal to 7.2 percent of the U.S. population. An estimated 84.1 million Americans aged 18 years or older have Prediabetes complications. In the European Region an estimated 60 million people living with diabetes, or about 10.3% of men and 9.6% of women aged 25 years are a diabetic patient. The 60% of the total diabetic population lives in Asia. Approximate 9 million adult people or 11.6% of the total China population is suffering from diabetes. In India, the number is 1 million and both countries will have half billion diabetic patients in 2030. There are 189 million cases of type 2 Diabetes Mellitus were recorded globally in 2012, with only 120 million of those cases actively receiving treatment, prevalence is expected to grow to 330 million cases globally by 2022. The global market is expected to achieve a sale of $85.9 billion up to 2022, supported by a CAGR of 5.2% during 2017-2022. In the U.S, the market yielded $16.4 billion for diabetes in 2012 and expected to grow up to $38.8 billion in 2022.
